What Does Shockwave Therapy Feel Like?

If you’re considering shockwave therapy for chronic pain, tendon issues, or soft tissue injuries, chances are you’ve asked yourself one key question: “But what does it actually feel like?”

Shockwave therapy—uses acoustic energy (yes, sound waves!) to stimulate healing in injured tissues. It’s commonly used for conditions like plantar fasciitis, tennis elbow, Achilles tendinopathy, to name but a few.

You walk in, likely a bit nervous if it’s your first session. The machine doesn’t look intimidating—it’s about the size of a small desktop printer, with a handheld device that resembles a chunky ultrasound wand. The clinician applies gel to the treatment area (cold and a little squishy), then presses the wand against your skin.

Does It Hurt?

Here’s the honest answer: it depends.

For most patients, the sensation ranges from mildly uncomfortable to quite intense—but it’s not sharp or stabbing. It’s more of a deep, repetitive tapping or pulsing, like someone is thumping your sore spot rapidly with their knuckle. The sound is a rhythmic clicking, like a woodpecker off in the distance.

Some patients describe it as:

  • “A weird buzzing pressure, like a jackhammer in slow motion.”
  • “Not painful, but you definitely feel it working.”
  • “A little stingy at first, then my body kind of got used to it.”

Pain tolerance plays a big role. If your area is inflamed or highly sensitive, it may feel sharper—but clinicians usually adjust the intensity level in real time. You’re in control, and it’s totally okay to ask for a break or lower intensity.

After the Session

Once it’s done (sessions are usually 3-5 minutes), you might feel:

  • A warm, almost flushed feeling in the area
  • Mild soreness, like post-workout muscle fatigue
  • Slight bruising or swelling (rare, but possible)

Shockwave therapy isn’t exactly relaxing spa-day material—but for many, the short bursts of discomfort are a small tradeoff for long-term pain relief. And the best part? There are no needles, no surgery, and very little downtime.
